PREVAIL Model for Prostate Cancer Survival
Predicts overall survival in chemotherapy-naive men with metastatic castration-resistant prostate cancer (mCRPC) receiving novel androgen receptor inhibitors

Why use it
mCRPC is a heterogeneous disease with highly variable survival, ranging from a few months to several years. The PREVAIL model provides: (1) an objective, validated survival estimate to guide individualized treatment planning and clinical trial eligibility; (2) a prognostic stratification that correlates with benefit from novel ARSi therapies; (3) a framework for goals-of-care and advance care planning discussions; (4) a tool for benchmarking outcomes in clinical practice against PREVAIL trial data; (5) identification of patients with poor prognosis who may benefit from early palliative care integration or escalation to combination therapies (e.g., ARSi + PARP inhibitor for HRR mutations).
About
The PREVAIL prognostic model was developed by Armstrong and colleagues and published in Annals of Oncology in 2018, derived from the landmark PREVAIL phase III randomized controlled trial (Beer TM, Armstrong AJ et al., N Engl J Med 2014). PREVAIL enrolled 1,717 chemotherapy-naive men with metastatic castration-resistant prostate cancer (mCRPC) who received enzalutamide or placebo, demonstrating a 71% reduction in the risk of radiographic progression and a 29% reduction in mortality with enzalutamide. Using baseline clinical and laboratory variables from PREVAIL, Armstrong and colleagues developed and validated a prognostic model predicting overall survival (OS) in men with mCRPC receiving novel androgen receptor signaling inhibitors (ARSi โ enzalutamide, apalutamide, darolutamide). The model incorporates eight pretreatment variables: ECOG performance status, lactate dehydrogenase (LDH), alkaline phosphatase (ALP), hemoglobin, PSA, presence of visceral metastases, number of bone metastases, and pain (opioid use). Patients are classified into three prognostic risk groups with substantially different median overall survival: Good (>24 months), Intermediate (12โ24 months), and Poor (<12 months median OS). The model was externally validated in the ARCHES and ARAMIS trial populations and is applicable to men initiating first-line ARSi therapy for mCRPC.
When to use
- Men with chemotherapy-naive metastatic castration-resistant prostate cancer (mCRPC) at the time of initiating novel ARSi therapy (enzalutamide, apalutamide, darolutamide)
- Prognostic counseling at mCRPC diagnosis โ informing patient and family about expected disease trajectory
- Clinical trial eligibility assessment and stratification for mCRPC studies
- Shared decision-making regarding treatment intensity, combination therapies, and palliative care integration
- Post-docetaxel mCRPC โ applies with reduced predictive accuracy; validated primarily in pre-chemotherapy population
- Assessment of candidacy for PARP inhibitor therapy (poor prognosis patients with HRR mutations may benefit most)
- NOT validated in high-volume visceral metastatic disease (rapidly progressive mCRPC) or in patients with prior ARSi exposure

Pearls & pitfalls
- LDH is the single strongest prognostic factor in mCRPC โ markedly elevated LDH (>2ร ULN) correlates with high tumor burden, glycolytic metabolism, and very poor prognosis regardless of PSA.
- Visceral metastases (liver, lung, adrenal) denote a biologically aggressive phenotype and independently predict poor OS even when burden appears limited on imaging.
- Hemoglobin < 11 g/dL at mCRPC diagnosis reflects both tumor burden and bone marrow involvement โ strong adverse prognostic marker.
- PSA is paradoxically unreliable as a prognostic marker in some mCRPC patients: PSA-low/LDH-high ("AR-indifferent" tumors) indicates neuroendocrine differentiation, which carries the worst prognosis and does not respond to ARSi.
- Opioid use at baseline (proxy for pain burden) is an independent adverse prognostic factor in the PREVAIL model, reflecting both quality of life and disease severity.
- The PREVAIL model applies to the novel ARSi era โ survival estimates are substantially better than older models derived from pre-enzalutamide mCRPC cohorts.
- All men with mCRPC should undergo germline testing for BRCA1/2, ATM, PALB2, and other HRR genes โ HRR-mutated patients are candidates for PARP inhibitors, which provide additional survival benefit regardless of PREVAIL risk group.
- PSMA PET/CT (gallium-68-PSMA or flotufolastat) is now standard for mCRPC staging in many guidelines โ PSMA expression impacts eligibility for lutetium-177-PSMA-617 (Pluvictoร), which improves OS in post-ARSi mCRPC regardless of PREVAIL risk.
- The model was derived before widespread use of systemic therapy in metastatic hormone-sensitive prostate cancer (mHSPC) โ patients who have received prior enzalutamide, apalutamide, or abiraterone for mHSPC will have fundamentally altered mCRPC biology.
- Bone metastasis number (assessed by conventional bone scan) โฅ10 was a threshold used in PREVAIL โ PSMA PET may detect more lesions; interpret bone scan burden in context of the imaging modality used.
Next steps
Good Prognosis Group (median OS >24 months): Standard ARSi therapy (enzalutamide, apalutamide, or darolutamide). Routine oncologic follow-up. Consider germline and somatic tumor genomic profiling for HRR mutations (BRCA2, ATM, CDK12) to identify PARP inhibitor eligibility. Bone health management (zoledronic acid or denosumab). Intermediate Prognosis Group (median OS 12โ24 months): ARSi therapy as above. More intensive monitoring. Early palliative care co-management. HRR genomic profiling prioritized. Consider combination ARSi + PARP inhibitor if HRR mutation present (olaparib + abiraterone, niraparib + abiraterone). Clinical trial enrollment encouraged. Poor Prognosis Group (median OS <12 months): Urgent palliative care integration. Rapid initiation of ARSi therapy. Combination approaches (ARSi + PARP if HRRm, ARSi + cabazitaxel or Lu-177-PSMA if PSMA-positive). Advance care planning. Goals-of-care discussion within 2โ4 weeks of diagnosis.
