Why use it

The IPSS-R is the universally endorsed standard for MDS risk stratification, required by NCCN, ELN (European LeukemiaNet), EHA, and BSH guidelines for treatment planning. It determines when to initiate active therapy (HMA), when to refer for allogeneic stem cell transplant, and how to counsel patients about prognosis and AML evolution. The five-category stratification provides greater prognostic resolution than the original IPSS, particularly distinguishing Very Low from Low risk (markedly different survival) and enabling nuanced transplant timing decisions. The molecular IPSS-M (2022) further extends the IPSS-R by integrating 31 gene mutations and should be used when NGS data are available.

About

The Revised International Prognostic Scoring System (IPSS-R) for myelodysplastic syndromes (MDS) was developed by Greenberg and colleagues and published in Blood in 2012. It superseded the original IPSS (1997) and represents the global standard for MDS risk stratification in clinical practice and research. The IPSS-R was derived from a database of 7,012 MDS patients from 11 international institutions and uses five weighted prognostic variables: cytogenetic category (the most heavily weighted), bone marrow blast percentage, hemoglobin, platelet count, and absolute neutrophil count (ANC). Patients are classified into five risk categories โ€” Very Low, Low, Intermediate, High, and Very High โ€” with median overall survival ranging from 8.8 years (Very Low) to 0.8 years (Very High), and time to 25% AML transformation from not reached (Very Low) to 0.73 years (Very High). The IPSS-R outperforms the original IPSS for predicting both overall survival and AML transformation, particularly in lower-risk disease, and is the primary basis for treatment decision-making โ€” including hypomethylating agent initiation, allogeneic stem cell transplant eligibility, and transfusion/growth factor support strategies.

When to use

  • Newly diagnosed MDS โ€” risk stratification at diagnosis to guide treatment decision-making
  • Re-evaluation at each bone marrow biopsy with updated cytogenetics and current CBC parameters
  • Pre-transplant evaluation โ€” identifying patients in higher-risk categories requiring early allogeneic SCT referral
  • Assessment of disease progression โ€” repeat IPSS-R when significant CBC changes occur or new cytogenetic abnormalities are identified
  • Clinical trial stratification โ€” IPSS-R is the standard stratification variable in all major MDS trials
  • Prognosis and goals-of-care discussions with patients and families at diagnosis and during disease course
  • NOT validated for therapy-related MDS (t-MDS), MDS/MPN overlap syndromes (CMML), or MDS after prior hypomethylating agent failure

Formula

IPSS-R Score = Sum of five weighted variables CYTOGENETICS: Very Good [monosomy Y sole, del(11q) sole]: 0 pts Good [normal; del(5q) ยฑ 1 other; del(12p); del(20q); 1 pt double including del(5q)]: Intermediate [del(7q); +8; +19; i(17q); other single/ 2 pts double independent clones not elsewhere classified]: Poor [-7 sole/double; inv(3)/t(3q)/del(3q) sole/double; 3 pts double including -7/del(7q); complex: 3 abn.]: Very Poor [complex: >3 abnormalities]: 4 pts BONE MARROW BLASTS (%): โ‰ค2%: 0 pts >2โ€“<5%: 1 pt 5โ€“10%: 2 pts >10%: 3 pts HEMOGLOBIN (g/dL): โ‰ฅ10: 0 pts 8 to <10: 1 pt <8: 1.5 pts PLATELETS (ร—10โน/L): โ‰ฅ100: 0 pts 50โ€“<100: 0.5 pts <50: 1 pt ANC (ร—10โน/L): โ‰ฅ0.8: 0 pts <0.8: 0.5 pts RISK GROUPS (Median OS | 25% AML Evolution): Very Low โ‰ค1.5: 8.8 yr | Not reached Low >1.5โ€“3: 5.3 yr | 10.8 yr Intermediate >3โ€“4.5: 3.0 yr | 3.2 yr High >4.5โ€“6: 1.6 yr | 1.4 yr Very High >6: 0.8 yr | 0.73 yr

Pearls & pitfalls

  • Cytogenetics is the most heavily weighted IPSS-R variable โ€” a very poor karyotype (complex >3 abnormalities) alone places the patient at Very High risk regardless of other parameters.
  • Bone marrow blasts >10% (now classified as MDS with increased blasts-2 per 2022 WHO/ICC) is the second strongest predictor and strongly drives High/Very High IPSS-R scores.
  • The IPSS-R was designed for UNTREATED, de novo MDS and should NOT be applied after hypomethylating agent failure โ€” prognosis post-HMA failure is significantly worse than IPSS-R predicts (median OS ~5โ€“6 months regardless of IPSS-R group).
  • Therapy-related MDS and MDS/MPN overlaps (CMML, RARS-T) were EXCLUDED from the IPSS-R derivation โ€” the score is unreliable in these conditions.
  • The IPSS-R does not incorporate molecular mutations โ€” the IPSS-M (Bernard et al. 2022, NEJM Evidence) integrates 31 gene mutations and reclassifies ~50% of IPSS-R Low/Intermediate patients to higher risk, particularly those with TP53 biallelic, RUNX1, or EZH2 mutations.
  • A single ANC <0.8 ร— 10โน/L adds only 0.5 points โ€” relatively low weight, but severe neutropenia independently drives infection morbidity and should guide G-CSF use regardless of IPSS-R group.
  • In the IPSS-R Intermediate group (score >3.0โ€“4.5), median survival is 3 years โ€” this is the most heterogeneous group and molecular testing is essential to refine risk.
  • Allogeneic SCT timing: NCCN and ELN recommend transplant for High/Very High IPSS-R and selective Intermediate patients โ€” ideally within 6 months of diagnosis for higher-risk groups to avoid disease progression.
  • ESA therapy is most effective in IPSS-R Low/Very Low with serum EPO <500 mIU/mL and transfusion burden <2 units/month โ€” response rate approximately 40โ€“60% in this population.
  • Luspatercept (activin receptor ligand trap) is approved for lower-risk MDS with ring sideroblasts (SF3B1 mutation) after ESA failure โ€” the COMMANDS trial (2023) showed superiority over epoetin alfa as first-line in transfusion-dependent patients.

Next steps

Very Low / Low Risk (IPSS-R โ‰ค3.0): Watch-and-wait for asymptomatic patients. Supportive care: RBC transfusions (Hb target 8โ€“9 g/dL), ESAs (darbepoetin/epoetin if EPO <500 mIU/mL), G-CSF for recurrent neutropenic infections. Lenalidomide for del(5q). Luspatercept for lower-risk MDS with ring sideroblasts after ESA failure. Reassess CBC every 3โ€“6 months. Intermediate Risk (>3.0โ€“4.5): Individualize based on comorbidities, molecular features, and patient goals. Consider HMA therapy (azacitidine, decitabine) for symptomatic or progressive cytopenias. Evaluate for allogeneic SCT in younger/fit patients with adverse molecular features. High / Very High Risk (>4.5): Active disease-modifying therapy strongly indicated. Azacitidine 75 mg/mยฒ SC ร— 7 days q28 days is the global standard. Urgent allogeneic SCT evaluation โ€” the only potentially curative option. Venetoclax combinations and IDH inhibitors for eligible patients.
