Why use it

Provides a simple, objective risk stratification tool that complements cytogenetic markers (del(17p), del(11q), TP53 mutation) in patients with R/R CLL. Identifies patients at highest risk of early treatment failure on BTK inhibitors. Guides shared decision-making about treatment intensity, timing of re-treatment, and clinical trial eligibility.

About

The BALL score is a prognostic scoring system developed for patients with relapsed or refractory (R/R) chronic lymphocytic leukemia (CLL) receiving targeted therapy (primarily ibrutinib or other BTK inhibitors). It uses four readily available laboratory parameters โ€” ฮฒ2-microglobulin, Anemia, LDH, and Lymphocyte doubling time โ€” to classify patients into risk groups with significantly different progression-free and overall survival outcomes. The BALL score was derived from real-world data of R/R CLL patients treated in the ibrutinib era and helps clinicians identify high-risk patients who may benefit from intensified monitoring, clinical trial enrollment, or earlier consideration of allogeneic stem cell transplantation.

When to use

  • R/R CLL patients initiating or already on ibrutinib or other BTK inhibitor therapy
  • Risk stratification at time of disease progression after first-line therapy
  • Identifying patients who may benefit from clinical trial enrollment or early allogeneic SCT consideration
  • Follow-up risk assessment in patients with CLL receiving novel targeted agents

Formula

BALL Score = Sum of 4 binary factors (0 or 1 each): B = ฮฒ2-microglobulin โ‰ฅ 5 mg/L: +1 A = Anemia (Hgb โ‰ค 11 g/dL): +1 L = LDH elevated (> institutional ULN): +1 L = Lymphocyte Doubling Time < 12 months: +1 Risk groups: 0: Low risk โ€” median PFS > 4 years 1โ€“2: Intermediate risk โ€” median PFS ~20โ€“30 months 3โ€“4: High risk โ€” median PFS ~12โ€“15 months

Pearls & pitfalls

  • ฮฒ2-microglobulin โ‰ฅ 5 mg/L is the single strongest prognostic factor in the BALL score โ€” it reflects tumor burden and renal clearance.
  • Anemia at the time of R/R CLL treatment reflects bone marrow infiltration and is a marker of advanced disease.
  • LDH elevation indicates rapid tumor proliferation and is associated with aggressive disease biology.
  • Lymphocyte doubling time < 12 months indicates rapidly proliferating disease โ€” this criterion may be difficult to assess in patients already on treatment.
  • The BALL score was developed specifically for the targeted therapy era (ibrutinib) โ€” its applicability to venetoclax-based regimens may differ.
  • Del(17p)/TP53 mutation and IGHV mutation status provide complementary molecular prognostic information not captured by BALL.
  • Richter transformation (transformation to aggressive lymphoma) is an important adverse event in R/R CLL โ€” BALL score does not specifically predict this.
  • The score should be recalculated at each new line of therapy as disease biology evolves.

Next steps

BALL Score 0 (Low risk): Median progression-free survival >4 years on ibrutinib โ€” continue targeted therapy; standard monitoring. BALL Score 1โ€“2 (Intermediate risk): Shorter PFS โ€” consider clinical trial enrollment; discuss allogeneic SCT if eligible; intensified monitoring. BALL Score 3โ€“4 (High risk): Poor prognosis on targeted therapy โ€” early clinical trial enrollment strongly encouraged; allogeneic SCT evaluation; consider venetoclax-based combinations.

Evidence

Developed from UK CLL Forum real-world ibrutinib treatment data. Published by Hillmen et al. as a validated prognostic index for R/R CLL. Patients with BALL score 0 had median PFS exceeding 4 years compared to approximately 15 months in score 3โ€“4 patients on ibrutinib. Endorsed by ESMO and IWCLL as a practical prognostic tool for the targeted therapy era.
