Why use it

Gold standard outcome measure in atopic dermatitis clinical trials and an important guide for treatment escalation. Required by regulatory agencies for biologics approval. Enables objective comparison of disease activity over time and between patients.

About

The Eczema Area and Severity Index (EASI) is a validated composite scoring system for quantifying the severity of atopic dermatitis (eczema). It assesses four signs โ€” erythema (E), edema/papulation (I for Induration), excoriation (Ex), and lichenification (L) โ€” each scored 0โ€“3, in four body regions: head/neck, trunk, upper limbs, and lower limbs. Each region is also assigned an area score (0โ€“6) representing the percentage of body surface area affected. The component scores are combined using region-specific multipliers to yield a total EASI score (range 0โ€“72). The EASI is the primary endpoint in major pivotal clinical trials for biologic therapies in atopic dermatitis (dupilumab, tralokinumab, lebrikizumab, abrocitinib, upadacitinib) and is widely used in research and clinical practice. An EASI-75 (โ‰ฅ75% improvement from baseline) is the standard trial endpoint.

When to use

  • Baseline severity assessment before initiating or escalating atopic dermatitis therapy
  • Monitoring treatment response (every 3โ€“6 months in clinical practice)
  • Clinical trial enrollment and endpoint assessment
  • Insurance prior authorization documentation for biologics (dupilumab, tralokinumab, lebrikizumab)
  • Determining eligibility for systemic or biologic therapy (moderate-to-severe: EASI > 16)

Formula

For each of 4 regions: Region score = (E + I + Ex + L) ร— Area ร— Multiplier Signs (each 0โ€“3): 0 = Absent 1 = Mild 2 = Moderate 3 = Severe Area (% BSA affected, per region): 0 = 0%; 1 = 1โ€“9%; 2 = 10โ€“29%; 3 = 30โ€“49%; 4 = 50โ€“69%; 5 = 70โ€“89%; 6 = 90โ€“100% Region multipliers: Head/Neck: ร— 0.1 Trunk: ร— 0.3 Upper limbs: ร— 0.2 Lower limbs: ร— 0.4 Total EASI = Sum of all 4 region scores (max = 72) Severity bands: 0: Clear | 0.1โ€“1.0: Almost clear | 1.1โ€“7.0: Mild 7.1โ€“21.0: Moderate | 21.1โ€“50.0: Severe | >50: Very severe

Pearls & pitfalls

  • Maximum possible EASI score is 72 (not 100). Head/neck has the smallest multiplier (0.1) โ€” even severe head involvement contributes โ‰ค7.2 points.
  • Lower limbs have the largest multiplier (0.4) due to their large BSA โ€” extensive lower limb involvement drives the total score significantly.
  • Each of the 4 signs (E, I, Ex, L) is scored 0โ€“3: 0=absent, 1=mild, 2=moderate, 3=severe.
  • Area score per region: 0=0%, 1=1โ€“9%, 2=10โ€“29%, 3=30โ€“49%, 4=50โ€“69%, 5=70โ€“89%, 6=90โ€“100%.
  • EASI-75 (โ‰ฅ75% reduction from baseline) is the FDA-approved primary endpoint for most AD biologics.
  • IGA (Investigator Global Assessment) 0/1 with โ‰ฅ2-point improvement is used alongside EASI as a co-primary endpoint.
  • The Objective SCORAD and NRS for itch/sleep are complementary patient-reported outcomes commonly measured alongside EASI.
  • Infants and young children: head/neck accounts for a larger proportion of BSA โ€” the EASI formula adjusts for this via multipliers.

Next steps

EASI 0: Clear (consider step-down therapy). EASI 0.1โ€“1.0: Almost clear (maintain current regimen). EASI 1.1โ€“7.0: Mild (optimize topical therapy โ€” TCS, TCI, crisaborole). EASI 7.1โ€“21.0: Moderate (consider dupilumab or other systemic; trial prior authorization often requires EASI โ‰ฅ 16). EASI 21.1โ€“50.0: Severe. EASI > 50: Very severe (urgent systemic therapy; biologics or JAK inhibitors indicated). EASI 50 reduction (EASI-50) and EASI 75 improvement (EASI-75) are key treatment benchmarks.
