Canadian Diabetes Risk Assessment Questionnaire (CANRISK)
Estimates 10-year risk of developing type 2 diabetes or prediabetes in Canadian adults
Calculator
Why use it
CANRISK is specifically validated for the ethnically diverse Canadian population, incorporating ethnicity-specific waist circumference cutoffs. A score ≥ 21 indicates high risk warranting formal blood glucose testing. Enables identification of high-risk individuals before symptom onset, when lifestyle interventions are most effective.
About
CANRISK is a validated, point-based self-administered questionnaire developed by the Public Health Agency of Canada to estimate the 10-year risk of developing type 2 diabetes or prediabetes in Canadian adults aged 20 years and older. It incorporates demographic, anthropometric, and lifestyle factors including age, sex, ethnicity, education, physical activity, dietary habits, family history, history of gestational diabetes or high blood glucose, hypertension, and waist circumference. CANRISK was developed specifically for the Canadian multicultural population and validated across diverse ethnic groups, making it more applicable to high-risk populations such as South Asians and Indigenous peoples.
When to use
- Routine screening in adults aged ≥ 20 years in Canadian primary care
- Identifying adults at high risk for type 2 diabetes or prediabetes
- Community-based diabetes screening programs
- Motivating lifestyle change discussions in primary prevention settings
- Before prescribing medications that increase diabetes risk (corticosteroids, antipsychotics)

Pearls & pitfalls
- CANRISK uses ethnicity-specific waist circumference cutoffs: lower thresholds for South Asian, Chinese, Japanese, Korean, Filipino, and Aboriginal/Indigenous Canadians.
- Gestational diabetes is a standalone high-risk feature — consider screening even with a low total score.
- Physical inactivity and unhealthy diet are modifiable risk factors — emphasize lifestyle modification regardless of score.
- South Asian ethnicity warrants a higher index of suspicion — diabetes can develop at lower BMI thresholds.
- Score ≥ 21 corresponds to approximately a 1 in 4 chance of developing diabetes or prediabetes in 10 years.
- The CANRISK has been validated with AUC 0.74 for predicting diabetes in the Canadian population.
Next steps
Score < 21: Lower risk — encourage healthy lifestyle. Rescreen in 3–5 years. Score ≥ 21: High risk — refer for formal blood glucose testing (fasting glucose, HbA1c, or 2-hour OGTT per Diabetes Canada guidelines). If prediabetes confirmed: enroll in structured diabetes prevention program.
Evidence
Robinson CA et al. (Can J Diabetes 2011) developed and validated CANRISK in 8,266 Canadians from the Canadian Health Measures Survey. AUC 0.74 for predicting undiagnosed diabetes/prediabetes. Score of 21 optimized sensitivity/specificity in the Canadian population.
